The ADA is developing a dental-specific standard (Proposed Standard 1120-5, July 2026) covering both X12 278 and FHIR for prior authorization and predetermination. Until payers adopt it, the packet with the X-rays and the narrative goes the way it has always gone.

Questions worth asking.
Claims and predeterminations largely move through the 837D transaction. What does not is everything attached to them and everything outside them: radiograph packets a payer requests by name, narratives for complex procedures, records requested by another practice, and Medicaid prior authorizations. Those are the pages that still need a compliant channel.
If you transmit protected health information, yes — and that requires a signed Business Associate Agreement with your fax provider.
